The Commission notified all applicants of its intent to award the grant funds to the following five highest scoring applicants:

• Beach Cities Health District (Los Angeles County)
• Peninsula Health Care District (San Mateo County)
• Sacramento County Behavioral Health Services
• Wellnest© (City of South Los Angeles)
• University of California – Irvine (hub) & Wellness and Prevention Center (spoke)
